documents that have places that you only have to add your personal information

TIPS ON SENDING
FORMS & LETTERS
When sending forms and letters to Agencies
1.) Have a Case Worker, Clergy, Friend, Teacher,
Parent, Lawyer, read the information for any errors.
2.) Have the letter NOTARIZED by a "NOTARY of
the PUBLIC" and have two (2) PEOPLE there with you to SIGN their NAME as
WITNESSES
3.) Now make three copies(3) each of all the
form and or letters. And give ONE (1) set of copies to a Case Worker, Clergy,
Friend, Teacher, Parent, Lawyer, to keep safe in the event something happens
to your sent.
4.) Now all of the the orginal forms and/or letters
take to the POSTOFFICE with the NAME & ADDRESSES to the PERSON YOU
ARE SENDING THEM TO.
5.) When mailing your form(s) and/or letters,
send them "CERTIFIED MAIL RETURN RECEIPT REQUESTED" & "CERTIFIED
MAIL RECEIPT." This means the MAILPERSON has the person SIGN THEIR
NAME ON THE LETTER and DATE. The MAILPERSON then mails the card back to
you.

RETURN RECEIPT REQUESTED CARD" card you need to fill out. The RED
is the places YOU FILL OUT.
This card is mailed to you when the Agency signs it when the Mailperson
delivers it to them.

MAIL RECEIPT" you need to fill out. The RED
is the places YOUFILL OUT.
This paper you get when you pay the Post Office for mailing. This is YOUR
PROOF OF RECEIPT until the card is signed and mailed back to you.
